Inadequate Requirements
The most common root cause of IT transformation failure: a solution that does not match what the organisation actually needed — because requirements were gathered from too narrow a stakeholder group, in a single workshop, without sufficient depth or validation.
How we address it
We conduct a structured requirements programme that engages all relevant stakeholder groups — operational users, technical teams, and executive sponsors — using facilitated workshops, individual interviews, and process observation. Underestimated Change Management
Technology programmes that treat user adoption as something that happens automatically once the system goes live produce technically delivered but operationally failed programmes — systems that exist but are not used, or are used in ways that undermine the value they were built to create.
How we address it

Compressed or Superficial UAT
User Acceptance Testing is the quality gate between a system built in a development environment and a system deployed in a live operational one. Programmes that compress UAT to meet a schedule deadline — or conduct it without structured test scenarios — deploy systems with undiscovered defects into live operations.
How we address it
We manage UAT as a formal programme phase — with structured test scenario design covering the full functional scope, tester preparation and briefing, tracked test execution, formal defect management, and a defined entry and exit criteria process. No deployment proceeds without a UAT sign-off that confirms the system meets the requirements it was built to fulfil.

Premature Post-Go-Live Withdrawal
The weeks immediately following go-live are the period of highest operational risk in any technology programme. Issues that did not surface in testing emerge when real users interact with the system under real operational conditions. Partners who treat go-live as the end of the engagement leave organisations exposed at exactly the wrong moment.
How we address it
We operate a structured hypercare period for every deployment — typically four to eight weeks of intensive post-go-live support — before transitioning to a normal support model. Hypercare is resourced at the same level as the delivery phase and includes daily issue triage, user support, and proactive monitoring for the indicators that precede adoption failure.
